6.  Took "Wellness" exam at Hospital. overall testing just see if all checked out.  They found a problem with my spirometer tests and suggested I see my doctor.  Almost at the same time, came down with severe bronchitis.. was going to Vegas so he gave me some inhalers and theophyline and said to come back on return.
7. Fortunately my Dr. was an internist and pulmonologist.
8.  Spirometer and "nuclear lung scan".  Breathed in gases and was observed through scopes.. etc.
9.  Same Dr. - He sat down with me and went to great lengths to explain my condition to made sure I understood.  I asked, on a scale of 1 to 10, with 10 being the worst, what my condition was.  I got a 5 and he explained about 50%.  The only improvement I could expect would be thru transplant.
10.  We started out pretty much with the basics.  Was put on Theo on "as needed" basis, along with some inhalers.  Kept about a 2-3 week follow-up until I was feeling better.
11.  Standards.... proventil primarily 2 puffs 4 times a day.  Also Atrovent.
12.  Proventil best, atrovent worst.
13.  Theophyline 200 mg 3 x day.
14.  This worked.
15.  Wellbutrin 200 mg 2xday, xanax 1mg as needed, usually take one in am

3. Never specifically defined - the pulmonologist hinted at emphysema and chronic bronchitis. I did have asthma as a child.

6. Could not breathe - PCP hadn't a clue, after prescribing several different antibiotics, inhalers, etc.
7. I insisted on being refered to a pulmonologist - after no improvement under PCP's care

10. Was that the arterial blood gas test and another blood test for theophylline?

12. Hard to say - Vanceril was discontinued, the inhaled albuterol and atrovent got changed to the nebulizer
13. Now taking theoplylline, 200 mg, 2x a day. Prednisone, 15 mg every other day. Fosomax, 10 mg, once in the morning for osteoporosis

9. Standard lung function tests--done at Highline Hospital and performed by the hospital's respiratory therapist. 
10.  I was not pretested for any medications.  I had been for testing by my family practitioner who then prescribed Atrovent as needed for rescue.  When I went to the pulmonologist, he added Aerobid (without a spacer).  A few years later, when I complained about increasing SOB, he prescribed Albuterol.  About five years ago he switched me to Flovent (he added the spacer at that time).  There were never any recommendations for exercise, pulmonary rehab, etc.  Occasionally, if I was having trouble with bronchitis or more than usual shortness of breath, he would give me a shot of prednisone.  He tried to put me on oral prednisone within the first two years of my diagnosis but I resisted.  I still have shots occasionally, but not for some time now.  A year ago I decided that the pulmonologist I had been seeing wasn't really interested in treating the disease effectively and made a change.  The new doctor, concerned about my frequent bouts of bronchitis, added Singulair and Serevent. 
11. Albuterol, 90 mcg. 2/4; Atrovent, 18mcg. 2/2; Serevent, 21 mcg. 2/2; and Flovent, 220 mcg. 2/2. 
12. I didn't like Combivent or Advair.
13. I currently take: Singulair, 10 mg., 1 tab. daily;  I also use Flonase and Allegra for allergies, hormone replacement therapy and Protonix for acid reflux disease.
14. As for effectiveness, I'm not sure how I would measure it.  I use the medications as prescribed and the disease just moves inexorably along.  Whether it would be better or worse because of a medication, I can't really tell.  Albuterol was becoming less and less effective as a rescue medication.  Dr. Jackson tried Advair and I just can't use that delivery system.  I don't have enough air when I inhale to get it down into my lungs.  It just stopped at the back of the throat and gave me a hum-dilly of a case of thrush.
15. I do not take anti-depressants or tranquilizers.  I'm generally pretty mellow but have to admit that when I had to start using oxygen I got a might worked up.  I belong to a 12-step program and have been practicing acceptance for nearly 20 years.  That helps.
16. At one point, the first pulmonologist prescribed prednisone and I took it for about one week.  I was so jittery and unpleasant to be around that I weaned myself off of it immediately and refused to take it from then on.
17. Neither of my pulmonologists has ever suggested rehabilitation or any specific exercise, other than telling me I should walk.
18. I have a nebulizer for use with Albuterol and Atrovent.  I was unable to see any improvement over my inhalers and since I lead a very active life, I put it on a shelf and dust it occasionaly.
19. I have been on oxygen for about a month.  I was visiting Mount St. Helens when I suffered from altitude sickness.  When I got to my Dr. she checked my O2 and put me on oxygen at 2L.  Two weeks later she reduced the dosage to 1L and said I don't need to use it when I'm at rest.  I have opted to raise it to 2L when I exercise.  She told me I didn't need it to sleep but when I got my oximeter, I checked it a couple of times during the night and it was down in the mid-80s, so I now sleep with it at 1L.
20. I am extremely sensitive to both heat and cold and have been for a year or so.  When the temperature goes up, I go down.  The cold just tightens me up so hard I can't breathe.  I'm learning to avoid both and fortunately, the Seattle climate may be damp but there aren't usually extremes of temperature.  As for incontinence, I've been using a double strength panty liner for a year or so.  I sleep like a baby, even with the hose in my nose.
21. I retired from my career as an educational administrative assistant in April of 2001 at age 62.
22. I am not on full or partial disability.  I have my retirement benefit and social security.
23. I belong to an HMO and so far, the coverage has been adequate.  I'm not sure what will happen in March 2004, when Medicare takes over.
24. I have not looked into LVR or lung transplants--YET.

The more I read on the EFFORTS site, which I just found when I was looking for information about oxygen, the more I wonder about the treatment I've had for the past 11 years.  If they had given me more information about the specific importance of exercise, told me more about my medications, etc., I wonder if I would be in better shape now.  Why isn't it routine for someone who labels him or herself a pulmonary specialist to prescribe some rehabilitation services when the diagnosis is made?  As for the corticosteroids, today's little tidbit about their lack of effectiveness for emphysema got my attention.  My son in law is a chiropractor and has maintained for 10 years that I shouldn't be taking them.  We've argued vehemently on the subject, as I maintained that a pulmonary specialist must know what he is doing.  Now I wonder.
27. I smoked for about 30 years and quit voluntarily when I was 45.  I thought I was home free.  :(

Since joining the EFFORTS team, I have begun a regular program of weight training, yoga stretches and using the stationary bicycle.  I have a treadmill in the basement and will start using it when we can't get outside anymore.  In my own defense, my husband and are are very active.  We shop, visit museums, tour around looking at new areas, usually on foot.  I haven't been "exercising," but I haven't exactly been idle.
29. To be honest, I don't remember the name of it.  My last upper respiratory infection has been nearly a year ago, about the time I changed pulmonolgists, and I think my PCP prescribed the stuff they're using for Anthrax.  Senior moment--I can't recall the name.
30. I have never been tested for Alpha 1.  My father died from complications of emphysema (my mother died of lung cancer).  Both parents smoked--but then, everybody did back then.

6. Shortness of breath that followed what seemed to be a very serious chest infection in 1990. The doctors were unable to identify what was causing this infection. I had a bronchoscopy and cultures done all to no avail. I didn't return to "normal" for almost four months!

blowing-into-machine test, without and with an inhalant; and a machine-on-the-finger test. My doctor is Board Certified in Pulmonary Medicine, but didn't sit me down and say: "You have Emphysema."  This year I intend to pin him down on the subject.

The ten year span between 1990 and 2000 showed very little change in my symptoms and because I stopped smoking such a long time ago, I wonder what the statistics might be in this situation.

28. I ride my bicycle nearly every day, sometimes for an hour or two--many miles, and I walk as necessary, probably at least six blocks a day. (Weather is the factor deciding my bike riding, walking I do despite everything else.) Sob during walking, not much during biking.
29. Had a bad chest cold in December and finally asked for antibiotic. Can't remember name, and not sure it helped. Cold ran its course.

5. Probably around age 50. I'd smoked since a teenager (quitting at age 70).  I'm sure cigarettes were destroying me over the
years but I'd rationalized that any shortness of breath was because I'd had dust pneumonia (Western Kansas Dust Bowl) at
age 13. I was also proud of my reputation over the years that I could out-work and play harder than almost anyone but I was
beginning to find at age 50 that I needed excessive amounts of oxygen for scuba diving (I still played tennis, polo and skied at
11,000 feet in Colorado).
6. Having just retired, decided I should start taking care of the body, particularly concerning my shortness of breath problem.
7. I obtained a pulmonary physician group recommendation from the local hospital.
8. A standard battery of tests to test lungs and heaert, x-rays, EKG's, stress test, oxygen monitoring when on treadmills, etc.
9. My pulmonary physician or designated hospital staff. Excellent analysis and communication.
10. Not to my knowledge.  There have been frequent changes in medication and dosages based on feedback from periodic
testing and/or discussion of my symptoms.
11. Flovent - 220 mcg - 3 puffs - 2 X daily
     Serevent - 13 g - 2 puffs - 2 X daily
     Atravent - 18 mcg - 3 puffs - 4 X daily.

7. After being ill with a lung infection for 3 mos. I had a stress related heart attack due to working with low oxygen levels. My emphysema was diagnosed at that time and that was when I started seeing my Pulmonologist.

9. Pulmonologist's in-office technician. My Dr. has state of the art computerized testing equipment in the office. I was fully informed of my condition, and understood as much as someone who is new at all this could.

12. I recently stopped using atrovent because of throat irritation and noticed no diference in how I feel so apparently it has been ineffective for awhile. Can think of no others.

16.Yes indeedy! Prednisone: puffy face, thinned skin; least bump would put large purple - red bruises and even tears in the skin on my forearms. Dry skin, thinning hair, weight gain, pred.induced diabetes, and other things I'm not sure about.
17. Scheduled for it a couple of times, but in 5 years became very ill and hospitalized 7 times with one heart attack, 2 intubations, and several near intubations. Arrested twice in the ER. Had long recoup periods after illnesses, so rehabs have been put on hold. Have equipment at home and use it but need to use it more (treadmill bike, healthrider). Have leveled out now and may soon enroll
again.

After going to the same pulmonologist for three years, a nurse at National Jewish Hospital in Denver suggested I be tested for Alpha1.  It was confirmed, and I was surprised that this "excellent pulmonologist" didn't think to have me tested.  But I have since found out that most people see at least three doctors before they are correctly diagnosed.   To this day I find that many doctors do not know of this disease, much less know what to do when they find it.
