5. Seems like I've always been SOB after exertion, climbing stairs etc. Once again attributed it to my weight. Have been between 250-325 lbs. since I was 15 yrs old other than a few dieting efforts.
6. 10 months ago mentioned it to Dr. He did a breathing test in office and said showed about the same level as test done a few years before. Emphysema was a problem, lungs were clear, as they always were. I went to Dr. every 2  months to get my sleeping/zanax pills refilled. Lungs always clear. 6 months ago, went to emergency with severe SOB and some chest pain. I was admitted for 8 days, in tough shape. o2 & co2 #s very bad. Did all tests possible, end result, severe Emphysema, will require o2 24/7.

1. James P. Endres, 1939, male
2. Littleton, Colorado, endresj@worldnet.att.net
3. Emphysema (Actually I have never been told what type and none is
indicated in my medical record.  Only: mild to moderate COPD and mild
obstructive Sleep Apnea). Diagnosis was July 1997
4. 57 1/2
5. 56 1/2
6. Swollen Legs, left was worse.  Unexpected and very rapid weight gain. Lethargy.  I was 56 1/2 years old, hadn't been to a doctor for more that 15 years and was doing well except for some problems with a numb area on my right thigh (later diagnosed as symptom of a degenerative lower spine).  I got a bad cold (upper respiratory problem), couldn't control it with cough medicine and realized that I probably had pneumonia.  Got antibiotic course from friend and took it as a doctor would have directed (and I quit smoking cold turkey!).  Got rid of the upper respiratory distress (cured the pneumonia) but didn't get to feeling better.  I was tired, lethargic and after two months noticed that I had gained 25 pounds in one month and had swollen legs, arms and face, much worse on my left side. Went to the doctor and got diuretics. Maximum doses, 7 or eight kinds, no, I mean NO effect. One year of this and got a new family doctor due to rotation at Kaiser.  He saw the swelling and confirmed with lung function and Oximeter a COPD
diagnosis.  Ordered oxygen for exercise and prescribed prednisone and Atrovent(r).  The oxygen service (Apia) sent a contract respiratory therapist who did a pulse Oximeter then did another after having me walk up and down a flight of stairs three times.  Told me (left nothing written) to use 4 or 5 lpm for exercise and, if I wanted I could use 2 lpm for light activity.  If I were you, he said, I would go ahead and use it all the rest of the time on 1 lpm.
7. Yes, three months later, after another change in primary care doctors due to retirement.
8. Well, just a Lung Function test on a spirometer and a walk around the office with a nurse and a Pulse Oximeter. The pulmonologist just read the medical record and prescribed a sleep study.  Then diagnosed the Sleep Apnea and prescribed 2 lpm Oxygen for sleep.
9. My family physician and I didn't get enough information because neither he nor I really knew what I was into.  I have gained what little I do know now mostly from self study and surfing the internet, visiting sites such as EFFORTS.
10. No
11. Azmacort(r) 4 puffs, 2 x day, Ventolin(r) (albuteral) 2-3 puffs every 4 hours, Atrovent(r) 3 puffs, 4 x day.
12. These are the ones I have used since first prescribed in August 1997. Have never tried any others.  I don't have asthma or bronchitis and have very little trouble with constricted ventilation.  However, I know if I miss my inhalers.
13. Predinsone for COPD. Levothyroxine 150 mcg per day for thyroid.
14. None other than prednisone taken for COPD.  The prednisone worked very well when taken for short periods (about a month) on two different occasions.
15. No and never have.  Although a Clinical Psychologist has diagnosed: Adjustment Disorder with Depressed Mood and Dementia Not Otherwise Specified.
16. Not really but did have a rash or rash-like breakout when taking heavy dosage of Lasix for leg swelling before diagnosis of COPD and Oxygen prescription.
17. No Re-Hab facilities suggested but Doctor instructed me to do exercise, "any kind just do it, it is important."
18. No, only MDI,s, although from time to time I consider changing.
19. Yes.  Full time, although I can take short trips into a store or to an event without it.  1-2 lpm when resting, 2-3 lpm light activity, 5 lpm heavy exercise, and 2-3 lpm sleep. Apria (through Kaiser contract) is Oxygen provider.
20. Heat & cold do not seem to bother me.  No incontinence.  I do have Sleep
Apnea diagnosis of 14 apneas per hour.
21. No

26. Patient knowledge and access to information about their diseases and ailments and everything related is a vital part of treatment.
27. No.  Former smoker for 40 years 2 packs a day (that's what I say but probably was closer to four, didn't keep count.)
28. Yes.  Exercise is essential to us with lung ailments, no matter how hard to accomplish - don't hurt yourself with it or without it.  I do strength (Total Gym), aerobic (Sky-Trek) & some specific therapeutic stretches for degenerative lower back at home.
29. Not much experience here.  I have only taken one episode of Amoxicillin (500 mg. 3x day x 10 days) self-prescribed, when I had pneumonia, and borrowed from a close friend but I worked well.  (When I had the pneumonia that set me on this final course and forced me into medical care and full disability.)  Two other courses of antibiotics were given at different occasions by doctor but precautionary and I saw no specific results unless I really had what they were trying to prevent.
30. No.
