My caregiver is my wife of 22 years (in May), Fern Mary Arenz Martini 3. A1AD and associated severe emphysema. 4. 45 on August 16, 1997 (A1AD) 42 in May of 1994 when diagnosed with "severe asthma and 'recurring' Bronchitis". In Thousand Oaks, Ca, at Kaiser Permanente facility. 5. Probably 1990 around that time I was having first sob problems and was told by a surgeon that I had heart problems. (This was later retracted by same Dr. after I quit smoking, EKG became "normal" 6 months later) 6. Shortness of breath (SOB) which was attributed to smoking 2 packs a day and an inguanal hernia. 7. No. 8. In 1997, I was given a lung function test, a full blood work-up and a lot of x-rays. 9. Dr. Lee Bevilaqua recommended Dr. John Connoly (who ordered the tests). I recieved enough information to be a danger to myself. 10. Never. 11. Flovent (220 mcg) 3 puffs B.I.D. Serevent (21 mcg) 2 puffs B.I.D. Albuterol (90 mcg) PRN 12. Atrovent was the least effective rescue inhaler, Aerobid the least effective steroid. 13. Very occasional prednisone. VERY occasional because I develop EXTREME manic/aggressive behavior, and have no idea as to why it's not funny to go from zero to POSTAL in .000001 microseconds. 14. A WWII era prescription for "allergies" . It's a combination of prednisone, multivitamns, beta blockers, and some herbs(and spices?:0) 15. Not yet. (Does it seem that I need them?? Someday I'll grow up, then the next day I'll probably die) 16. Adverse reactions to Aleve(sp?) and PREDNISONE. 17. Yes, Pulmonary re-hab, after Pneumonia in April of 1998. Walking, bicycling, MINOR weights. 18. Occasional. In Md's office. Albuterol. 19. 24/7 2.5 lpm for sitting/sleeping 3 lpm for getting around outside. 4 lpm on the treadmill. 20. Intolerances : Humidity above 55-60% (I grew up in CA dessert areas): Cold below 35 degrees is absolutely debilitating to me. 90% of all industrial/individual perfumes. 21. Not employed. 22. Just started the SSD trail in December 1998. 23 Prior to Jan 1,1999, HMO. PPO Jan1, 1999. HMO was not bad. Don't know about PPO yet. 24. Not yet. 25. Yes. 26. Haven't I made enough?? UH.. EDUCATION would have saved me a lot of this trauma, my brother was told that A1AD was magic or something, that there was no treatment, no relief besides O2, and that neither his kids or siblings could get it. Did a 6 Week rehab at local hosp. Upper body,treadmill,bicycle,etc. 18 Yes Use proventil & atravent 4 times a day.Use a proventil inhaler in between when necessary. 19 Yes Use oxygen all night and during any exertion. Use when on treadmill or walking anywhere. 20 Can't take a very hot shower.extreme heat or cold is bad 21 Yes 22 Am employed part time in as Jury Comm. for court. (Looking over the text below, I seem to have answered most of your remaining questions but not in their order. If this is unacceptable, please advise). For some years I had undergone attacks of bronchitis lasting for from one to three months. Considerable mucus formation requiring constant clearance by sharp exhalation of breath. This was noted by my GP in my annual checkups, but since there was no noticeable SOB and chest x-ray was negative, I was usually given an antibiotic which seemed to work with the standard advice to give up smoking (which had been given by various doctors for the past 35-40 years). . Two years ago (1998) I began to notice some difficulty breathing. However, I rarely exercised, and assumed this was due to being out of shape. I gradually reduced my cigarette consumption from a pack (of the ultra lights - I could not inhale regular cigarettes without gagging) a day to about half that. However, I continued to smoke cigars (usually once a day after dinner - I rarely smoked cigarettes in the evening) and a pipe. My bronchitis cleared up about two years ago - no mucous, no coughing, not even the flu. In my latest checkup in June of 2000, my GP referred me to a pulmonary specialist (PS). I immediately gave up the cigarettes. In September, I was diagnosed as having emphysema - borderline severe. Combivent brochodialator was prescribed (two puffs four times daily). I have ceased smoking altogether (last smoke was 8/11). To be truthful, quitting smoking was not a big problem. In these circumstances, I figured I had had a lifetime ration of tobacco smoke. In addition, oxygen therapy was prescribed 24/7 at a flow of 1 lpm. If I am spared any other illnesses, I feel confident that I can arrange my life and lifestyle to accommodate LTOT. I am investigating portable concentrators, among other things. As stated, I will be
77 on 9/4/00. I am about 60 lbs overweight (at 5'11'' I weigh about 245
- and the weight has crept up since I ceased smoking). The PS provided
me with an informative illustrated booklet on COPD. I searched the net
for further information and found quite a bit. I have now read a number
of articles about COPD focusing on emphysema and have learned quite a
bit from these sources as have found the postings to the EFFORTS
listserv enormously informative and interesting. I purchased an oximeter.
When I am resting comfortably I get Sp02 readings from 93-95. I have led
a very sedentary life for many years, but since my diagnosis have
resolved to get some exercise daily. I can now walk for between 15
minutes without getting winded (providing the weather is not too humid,
as it often is here in the summer) and resolve to improve this. I will
now start to attack the weight problem. I feel much better than I did a
month ago when many relatively minor physical tasks would leave me

Initially diagnosed with Chronic Bronchitis at 30, Emphysema at 42 (Acute Asthma attacks in between) 5. Emphysema - SOB at 40 (this was my period of self denial) 6. In the beginning the Uniphyl was making me sick, so they cut my dosage down from one whole one twice a day so they cut the dosage in half and I began taking half a pill twice a dy and I was no longer sick. 13) Atrovent Nebulizer or puffer 2 puffs every 6hrs Albuterol Nebulizer or puffer 2 puffs every 2 to 3 hrs as needed Cardizem CD 120mg 1 time a day Flovent 2 puffs 2 x's a day Guifed 250mg 2 x's a day Ibuprofen 600mg every 6hrs as needed for pain. Pepcid 20mg 1 tablet 2x's day Rocaltral 0.25 mg 1 tablet once a day Theophyllene 200mg 1 ablet 2x's a day Vitamin E 400mg 1 time a day Prednison 5mg once a day (now) 14) Uniphyl 15) No, I took Lorazepam 0.5 for a few days and my daughter told me to get off of it I had no expression or emotions. 16) No, I had a little trouble with Unyphil at first it was too strong but once we cut the dosage down it's been fine. Maybe we could say that the Pednison has caused me some problems I have degenerative arthiritis in my shoulder and cateracts in both of my eyes and they are all caused from steroid. 17) I tried therapy 2 years ago, ended up worse for it and was hospitalized, I tried again this past spring unfortunatIy ended up nthe hospital again. So I try to keep myseff busy around the house walking and doing the little bit of arm exercises that I can. 18) Yes, I use a nebulizer an Invacare-Enoy I use Atrovent and Albuterol. 19) I use OX 24-7 at 2 liters. 20) I have trouble with heat I can take the cold better. My room is usually freezing with muliple fans going to circulate the air I usd to have sleeplessness until I started using m bi-pap. 21) I had to give up my part time job last year; it involved too much walking and getting in and out of my car. 22) I am on full disability and have ben for 3 years now but I was allowed my little part time job . 23) I have Medicare and Medicaid so far everything has been paid for but I don't know how I would manage if I didn't have the Medicaide. 24) I am in the NETT program for Lung reduction and also on the list for a Transplant. 25) I can always be reached at this E-mail adress. 26) I am up and down so much... I just hate losing my independance . 27)I wonder about the doctors,,,the pulmonologist at University of Pitt. said that I wasn't a good candidate but the surgeon seems to think that I would be perfect???? I' m gonna have to go for 6 weks straight. 29) I'm not having much luck with anti-biotics it seems I take one for a week then I' OK but a week later I need another one , and since I'm a pink puffer I can't cough anything up so it just sits down there and keeps infecting me.
