Yes Zoloft and Desyrel 16. Acid reflux from the mixture, also fat from prednisone!! 17. Yes I go to rehab somewhat regularly, treadmill, bike, rowing machine, ergometer. Also some band work. 18. Yes I use a nebulizer when I'm not getting good reactions from the MDIs. I use a Devilbiss and I also have a micro battery operated one. 19. O2 2L 24 Liquid 20. temperature intolerance to both heat and cold. Incontinence during extreme SOB periods. Muscle weakness and fatigue. 21. I am self employed and have not fired myself yet. I'm quite lenient about working hours, fortunately I have some very good employees and a great business partner. 22. Not yet. 23. Private Insurance, so far excellent coverage. 24. I was in program at U of M, but they dicided my disease was too diffuse to be helped by LVRS. 25. I will be more than happy to help anyone I can anytime I can. 26. associated with Emphysema
6-I had gone to Canada, to a winter carnival and had a hard time to breath but
at first I thought that it was because of the very cold weather

8-The first time that I went to a breathing doctor, he gave me a tube or
something to blow in then he said I needed inhalers and gave me a prescription
for atrovent, then later to combivent
9-The first time it was a test in the Dr's Office then I had others at the
hospital via computerized repports, and no I really do not feel that I
received proper info, because I had stopped smoking long before I started
these test.
10-, No I do not think that I received any test relative to the medication,
however, I did have to give sputum samples and as a result
it was determined that I had yeast infection on my lungs, and I was given
medication for that, also levoquin andfluconazole
11-, Atrovent, flovent and combivent, all 2 puffs 3 times daily or as needed.
12-Maybe flovent, combivent is the most helpful, I have tried advair but it
does not work good with me.
13- Theodur 24, Levoquin both one a day.
14- Biaxin , I was told that my type of emphysema, they had tried 21 different
type of antibiotics and only levoquin proved best.
15- No I do not need anti-depressants and or tranquilizers although at times
when I get an attack, I do get nervous and I think maybe it would help.
16- Only one that I know of I beleive it was Zythromas, intovenous and my arm
got real itchy and it swelled-up
17- I went through a 10 week pulmanory therapy, and some information and
exercise did help.
18-Yes, I have two nebulizer one downstairs and another up stairs and use
albuterol 2.5 Ipratropium 0.5mg 3 mi comp.also
I use Mucous mixture to loosen phlim once or twice a day.
19- I was using oxygen 24 hrs a day, and have been using it since October, by
way of nose canula's, however, two weeks ago my nostrils had dried-up so much
that my nose started to bleed, and did so for 6 hours, neither we here at home
could stop it, the rescue came and they could not do anything and was
transported to the hospital and after 6 hours a specialist doctor cauterized
it however I required 2 blood trandusion.
20-Just what I stated above and yes cold weather bothers me.
21- I am a retired police officer, I was just elected to the School Board here
in Woonsocket, however I am involved with veterans affair and fraternal and
vet associations.
22- I do not think that I am fully disabled, but restricted somewhat.
23-I have medicare and Blue Cross Plan 65
24-No, do not know much about that
25-The reason that I said that I would serve as a chapter Leader is because I
want to help out and keep me busy, I do work at time on a volunteer basis at
the VA Hospital in Providence.
26-As I help others if I can, I will be helping myself, I know that I have met
some very nice people through EFFORTS and have learned a lot.
27, I gave up smoking 25 years ago.
28, I do nmot exercise as I should but I am retricted due the fact that I had
3 cervical disk problem which were operated on, two rotator cup operastion and
need one now but I have delayed it because of my breathing and probably will
have to be operated on within the nect couplr of weeks, but I am still very
active and I feel that what I do gives me the exercise enough.
29-Levoquin
30- I really do not know what is the testing for alpha1 genetic emphysema, but
write me and let me know and I will check with my Doctors.

COPD , some chronic bronchitis but mainly severe emphysema 4. emphysema 45, COPD 50 5. chronic smokers bronchitis 40, major s.o.b. 50 ish 6. smokers cough and bronchitis 7. not at first, then for alpha 1 testing, then as a result of severe emphysema then for alpha 1 , LVRS, and TX evaluation. 8. chest x-ray, complete pul.function tests, a blood gase's., CAT scans, nuclear lung scans, high resolution lung scans, pul.-rehab. testing; 6 min walk etc. FEV1 15% of normal "severe obstructive lung defect" "there is a severe decrease in diffusing capacity". ( June 2000 test) 9. a.PCP's, Gen Pract. Docs, Pulmonologist, RT's, X-Ray Techs, RN's b. NO! "ask your doc" responses only, self study and research to ask correct question. 10. only for "bronchodilators" via peak-flow testing. All other meds were prescribed and seldom monitored for effectiveness. 11. Flovent 110, 2 puffs twice daily, Albuterol with Atrovent nebulizer every 4 hours 4 x daily, Combivent inhaler as a (back-up bronchodilator) 2 puffs prn ( when necessary) 12. (It took many months of trial and error and my evaluation by very slowly discontining to learn if they were really effective for " emphysema": they were Alupent), Serevent , Aerobid, & Azmacort, 13. HCTZ 12.5 mg. (one tab daily) and one low adult strength Aspirin ( 81 mg) 14. Theophylline- Theodor, Voltex, Prednisone, Zoloft,Buspar, Proscar, Doxcycl, Cardura,, Hytrin, Orasone, Theochron, Zithromax, Acetamnophen, Biaxin,Provental Retab, 15. a. yes and b. Xanax to counter Albuterol anxiousness on occasion 16. Side effects not "reactions" of Corticosteroids which are many including 60 lb. weight gain, cataracts, and more; which are not really "adverse reactions" 17. a. Pulmonary Rehab srongly suggested,and b. dailyTreadmill as a minimum. 18. Yes, a "Pulmo- aide " both in home and portabnle. Albuterol with Atrovent 19. a.Yes and b.
