16.  Slight tremor from the Theodur, but no other noticed reactions.
17.  Yes. I have completed an 8 week pulmonary rehab course and have nothing but positive things to say about the experience.  It helped me to understand and to cope better with my condition. I highly encourage everyone to attend rehab if they can.
18.   Yes.  I do the Albuertol and Atervent mixed and its 4 times a day.
19.   Yes. I am on 24/7.  I have a concentrator at home and have 40 ft
of hose on it.  MY home settings are 2.5 at rest and 4+ with activity.  I have a Pulse/Dose portable that uses the Oxylite tanks(cylinders). I use that at 2.0 at rest and  5.0 with activity.
20.   Yes. when I am in heat and exerting effort I get into distress real fast and have to cease and rest. On the other hand cold seems to give me endurance somewhat.  I can exert longer and not have to rest a often. I have incontinence a lot more that I would like to, but am able to control it to a point.  I try to look as far ahead as possible and not get into situations where I lose control.
21.    Yes, but let me explain.  I am retired from Bellsouth(1996) and self-employed as a Distributor for Home Interiors & Gifts.  The job has me active for 3-4 days a week, and I have a partner that does the labor. I do the paper work.
22.   No
23.   I am covered by a PPO from Bellsouth.  It is not near sufficient for the needs of this disease and it would seem that they are not going to change.
24.   Yes,  I was accepted at Shands as a candidate in October of 1998.They discovered that I have a partial blockage in my aorta and that I have a decreased heart function, explained as: to many years of over work due to the effects of the cigarettes . I am now seeing a heart doctor as well.
25.   Yes,  Anytime that I am home I would be more that glad too talk to anyone. One of the things that I feel very strong about is boosting up someone's confidence in their MD's and in the current procedures and personnel in the pulmonary field.
26.    I would like to be able to stop all young people from smoking at all.  I know that that is an impossible dream, but smoking is such a sneaky demon.  It can and does ruin all things that we all look forward to for our waning years. If I could just open some minds a little to the "down the road" things young people that smoke have to look forward to and spare them of having to plan things , not with your family, but with your appliances and your family. I think that would be a answer to prayer.
27.    Yes (35 YEARS)
28.     No not on a regular basis.  I do get a lot of exercise on my job, but not as taught in the pulmonary class.
